Uses and Indications :

ANXO PR is a combination medication used to treat a variety of conditions, primarily related to anxiety and panic disorders, and it can be prescribed for the following:

  • Generalized Anxiety Disorder (GAD): Alprazolam helps manage the symptoms of anxiety, while Propranolol can help with physical symptoms like tremors and rapid heart rate.
  • Panic Disorder: It is effective for people who suffer from panic attacks, as it combines the calming effects of Alprazolam with the heart rate-regulating properties of Propranolol.
  • Situational Anxiety: Propranolol is often used to help control the physical symptoms of anxiety (such as rapid heart rate and tremors) during stressful situations like public speaking.
  • Performance Anxiety: Used to manage physical symptoms of anxiety when individuals need to perform in front of an audience or in high-pressure situations.

Mechanism of Action :

  • Alprazolam (0.25 mg): Alprazolam is a benzodiazepine that works by enhancing the action of gamma-aminobutyric acid (GABA) in the brain. GABA is an inhibitory neurotransmitter that reduces neural activity, resulting in a calming, anxiolytic (anti-anxiety) effect. It helps to reduce symptoms of anxiety, panic, and stress.
  • Propranolol (20 mg): Propranolol is a beta-blocker that works by blocking the effects of adrenaline (epinephrine) and norepinephrine on the heart. This leads to a decreased heart rate, reduced blood pressure, and reduced physical symptoms of anxiety such as shaking, sweating, and tremors. It is commonly used to treat hypertension, arrhythmias, and performance anxiety.

The combination of these two drugs helps address both the psychological and physical components of anxiety and panic disorders.

Dosage :

The dosage of ANXO PR should be individualized based on the patient's specific condition and response to treatment. Typical dosage regimens might be:

  • Generalized Anxiety Disorder (GAD) and Panic Disorder: The initial recommended dose is typically 1 tablet (containing 0.25 mg Alprazolam and 20 mg Propranolol) taken 1-2 times a day, depending on the severity of symptoms.
  • Situational Anxiety (e.g., public speaking): Take 1 tablet about 30-60 minutes before the stressful event or situation.

Side Effects :

Contraindications :

  • Breastfeeding: Both drugs pass into breast milk and could harm a nursing infant.

Narrow-angle glaucoma: Propranolol should be avoided in patients with this condition.

Drug Interactions :

Several drugs may interact with ANXO PR, and these interactions could either enhance side effects or reduce the effectiveness of the medication. Some important interactions include:

  • CNS Depressants (e.g., alcohol, opioids, sedatives, and other benzodiazepines): Can increase the sedative effects, leading to drowsiness, respiratory depression, or overdose.

  • Antihypertensive Drugs: Since Propranolol reduces blood pressure, taking it with other blood pressure-lowering medications could cause hypotension (low blood pressure).
  • CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, grapefruit juice): May increase the blood levels of Alprazolam.
  • CYP2D6 Inhibitors (e.g., fluoxetine, paroxetine): Can increase the concentration of Propranolol.
  • Other Beta-blockers: When combined with other beta-blockers, Propranolol can cause excessive slowing of the heart rate.

Always inform your healthcare provider about all medications, including over-the-counter drugs, herbal supplements, and vitamins you are taking to prevent harmful interactions.

Warnings and Precautions

  • Dependence and Withdrawal: Prolonged use of Alprazolam (a benzodiazepine) can lead to physical and psychological dependence. It is important not to abruptly stop taking the medication; the dose should be gradually reduced under a doctor's supervision.
  • Elderly Patients: Caution is advised, as older adults may experience more pronounced side effects such as dizziness, falls, and confusion.
