Indication/ use of Produlose Solusion :

Lactulose solution is primarily indicated for the treatment of constipation and hepatic encephalopathy.

Mechanism of Action of Lactulose Solution :

Lactulose is a synthetic disaccharide that is not absorbed in the small intestine. It is metabolized by colonic bacteria into lactic acid and other organic acids, which increase the osmotic pressure in the colon. This process draws water into the bowel lumen, softening stools and promoting bowel movements. Additionally, for hepatic encephalopathy, lactulose reduces the absorption of ammonia by converting it into ammonium, which is less readily absorbed, thereby lowering blood ammonia levels.

Dosage of Produlose Solusion:

Constipation:

Adults: Initially, 15-45 mL orally once daily, adjusted based on response. Maintenance dose is typically 15-30 mL per day.

Children (7-14 years): 15 mL orally once daily, adjusted as necessary.

Infants and children (1-6 years): 5-10 mL orally once daily.

Infants (less than 1 year): 2.5-5 mL orally once daily.

Hepatic Encephalopathy:

Adults: 30-45 mL orally 3-4 times daily, adjusted to produce 2-3 soft stools per day.

Alternatively, it may be administered rectally in a diluted form (300 mL of lactulose solution in 700 mL of water) as a retention enema.

Side Effects of  Lactulose Solution:

Common: Flatulence, bloating, stomach pain, nausea, diarrhea.

Serious: Severe dehydration and electrolyte imbalance (with excessive use).

Contra-Indications of Produlose Solusion:

Patients requiring a low-galactose diet (due to the presence of galactose in lactulose).

Hypersensitivity to lactulose or any component of the formulation.

Patients with gastrointestinal obstruction or perforation.

Drug Interactions of Produlose Solusion:

Antibiotics: Broad-spectrum antibiotics may reduce the effectiveness of lactulose by decreasing the number of bacteria that metabolize lactulose in the colon.

Antacids: Concomitant use with antacids can reduce the efficacy of lactulose.

 

Lactulose solution is a well-established treatment for constipation and hepatic encephalopathy, leveraging its ability to increase stool water content and reduce blood ammonia levels. While generally safe, it must be used judiciously to avoid significant side effects and interactions, especially in patients with specific contraindications.
